Application for a permit in respect of a particular stage carriage

The Regional Transport Authority,

Kashmir/Jammu.

 

In accordance with the provisions of section 45, 46 and 57 of the  Motor Vehicles Act, 19, 39.

 

I, the undersigned hereby apply for a permit under section 42 of that act in respect of a stage carriage as hereby  set out:-

 

1.         Full Name

2.         Name of father (in case of an individual)

3.         Address

4.         The route or routes or the area for which the permit is desired

 

5.         No. of vehicles proposed to operate in relation to each route or area

6.         The type of Vehicle

7.         Approx. seating capacity

8.         A time table is appended

9.         The standard rates of fare which it is proposed to charge is paise  passenger per kg.

10.    The minimum and maximum No. of daily services proposed to be provided in relation to each route

11.    No. of vehicle kept in reserve to maintain the service regularly and provide for special occasions

12.    Arrangements made for housing and repair of vehicles

13.    Arrangements made for convenience and comfort of passengers

 

14.    Arrangements made for storage and safe custody of luggage

15.    Particulars of any stage carriage or contract carriage or public carrier's permit valid in the stage and held by the applicant including his family in respect of (1) this vehicle             (b) any other vehicle

16.    Particulars of any permit held by the applicant in  respect of the use of any transport vehicle in any other state. Whether any other permits stated above has been subject of an order of suspension of cancellation in last four years, if so give details

17.    (i)        I desire to use the vehicle for the carriage of goods and I am for a 
           public/private carrier's permit in addition to a stage carriage permit
           in respect of route/routes or area above specified

ii)                  I intend to carry goods of the following description of use

18.    I desire to use the vehicle as a contract carriage within the area specified below:-

19. (i)  enclose ....................... being the prescribed fee
(ii) I am in possession of the vehicle the certificate of registration of which is enclosed

20.    I have not yet obtained possession of the vehicle and I understand that the permit will not be issued until have done so and have produced the certificate of registration

21.    I intend to drive the vehicle

22.    I desire a permit valid for                                              years

23.    I hereby declare that the above statements are true and agree that they will be conditions of any permit issued to me
